![]() However, others did not know how to use any of their MDI devices correctly. Much of the problem centered around patient misinformation about treating their disease.įor example, RTs had identified patients using multiple rescue inhalers in a row before finally deciding to seek care at the hospital. Part of their rationale for the policy was that if the patient’s MDIs were working as they should, the patient most likely should not have ended up in the hospital with an acute exacerbation. “Otherwise, the home med list of MDIs is converted by pharmacy physician-approved protocol to the nebulizer equivalent.” “Our policy was all patients will receive nebulizer treatments while in the facility unless the patient brings their home MDI and the physician agrees to continue that use of said MDI,” said Matt Nolan, MBA, BSRC, RRT, RRT-NPS, RRT-ACCS, director of cardiopulmonary and neurodiagnostics. Texas Health Resources Southwest Fort Worth was already delivering respiratory treatments primarily by nebulizers before the pandemic hit. None of the cases were linked to patients who received AGPs under our care.”Īdvertisement Sticking with the status quo “Only a few of our staff have had COVID over the past year. “We provided extensive education with our RT, nursing, physician, and leadership about how we were safely and effectively administering nebulized medications,” Caraway said. Staff in and out of respiratory care were well educated on the factors involved as well. Patients received their nebulizer treatments in negative pressure rooms, and additional rooms were added as COVID-19 cases surged in their area. In addition, all nebulizers were breath actuated, and each nebulizer was filtered. In their case, they ensured all staff members had adequate PPE, including N-95s, gowns, and PAPRs. ![]() “To date, all medications are nebulized at our facility, with a few exceptions, including patients on our behavioral health unit and patients who refuse nebulizers and wish to provide their own home MDI.”Ĭaraway believes nebulization can be performed safely in hospitals with the proper precautions. But, ultimately, it was decided that we needed to do what we felt was best for our patients,” she said. “Some physicians and nursing staff voiced their concerns about these aerosol-generating procedures and others such as the use of high flow oxygen. ![]() By October of last year, the hospital reinstated the nebulization option.Ĭaraway says she is proud of her staff for stepping up to speak for patients in need, even when others in the hospital were still wary of the change. Nebulization was considered a better option for these patients, and the staff went to bat for their patients in advocating for a return to the previously decided-upon policy. “Many of our regular COPD patients were not relieved of their shortness of breath and did not have the strength and/or coordination to activate the inhalers they were receiving,” she said. “In the beginning of the pandemic, we worked with pharmacy to order additional MDIs and transition all medications to non-aerosolizing medications in an effort to protect our staff,” Caraway said.Īs the months wore on, however, the general feeling of the RT staff was that MDIs were not delivering enough medication to patients who needed it. The decision to switch to nebulizers came from a multidisciplinary team that reviewed new medications, processes, and options on the market. The common canister program was implemented about eight years ago as a cost-saving measure. “Prior to the pandemic, we used common canisters for MDIs and also administered nebulizers,” said Lexie Caraway, MBA, RRT, RRT-NPS, RRT-ACCS, AE-C, manager of respiratory therapy. The Sarah Bush Lincoln Health System in Mattoon, IL, was beginning to explore the switch to nebulization when the pandemic hit. Is it safe to use nebulizers in the hospital setting during a global pandemic spread by respiratory means? Three RTs share their experiences. Now with more than a year of pandemic experience under their belts, some hospitals are rethinking that move. As a result, many hospitals that had previously switched from MDIs to nebulization for in-patients to improve care and save on costs discontinued those programs. Nebulization has particularly been called into question. Still, for respiratory therapists, the debate surrounding aerosol-generating procedures (AGPs) has probably been one of the biggest. The pandemic has raised many safety issues related to the delivery of patient care. Taking notes is essential, since information that's relevant is spread all over the map. This time there's fewer of them, but they're far bigger and more difficult to solve. There's a ton of environmental story telling, and since you'll be paying attention to every little detail for the sake of the puzzles, you'll end up becoming very familiar with its world and characters. The story is a big improvement over the first game, with a much more compelling villain, and clearly defined character motivations. Something like this would make it far less frustrating for future players.īesides those complaints, there's a lot that's great about this game. I'm kinda tempted to put together a guide which lists the locations of all the easily missed objects, without spoiling any of the puzzle solutions. Every time I got stuck, I would have to check around the whole world multiple times looking for interactable objects that I had missed. I got through the whole of Myst without walkthroughs, but I had to check them multiple times in Riven because I had missed the location of a button, lever, or passage. The big problem with the inability to move freely is it makes it so much easier to miss things. I will say that once I'd gotten used to this, I didn't think it affected the atmosphere much, which is still excellent, but it significantly hampered my enjoyment of the puzzles. The downside here is that there are no remakes, so you'll be stuck clicking your way around pre-rendered images of the islands. I haven't seen puzzles exactly like this in a game before, and it had me taking notes about everything I came across, using all parts of my brain to try to put the necessary bits of information together.Īfter you've played Myst, if you enjoyed it, I would also recommend giving Riven a try. There's a reasonable amount of variety, but most involve finding some kind of big complex machine without an instruction manual, and having to figure out how it works, and how to get it to do what you want it to do. If you're anything like me, this will keep you interested till the end. There's a lot that's unclear about the story at first, and pieced together as you play. The atmosphere is thick and mysterious, with great sound design that really helps bring the world to life. It has some remakes which allow you to move around freely, making it much more playable for a modern audience. Myst, in particular, I would recommend to everyone. They've hung around in my thoughts a lot since then though, and people don't post about them often here, so I think I ought to give a small recommendation. This method, originally developed for rigid suspensions, was extended to deformable red blood cells (RBCs), particles, capsules and fibers. The LB-FE method resolves the fluid phase, both interior and exterior to the fluid-filled particles, with the LB method. A recent review article highlights the feasibility of using the LB method for complex flows including suspension of deformable, particles, capsules and fibers. We examine the dynamics of deformable capsules and how these dynamics affect the rheology of dense suspensions using the LB-FE method. Of particular interest are the particle pressure and a full characterization of normal stresses, which are difficult to measure experimentally. The following figure demonstrates a novel problem where the particle microstructure affects the viscosity, normal stresses, and particle pressure of a suspension. The maximum viscosity and first normal stress difference is obtained when particles are aligned in a cluster along the compressional axis. Red blood cells (RBCs), the most numerous constituent of blood, influence continuum-level measurements by altering the suspension at microscopic scales. In moderately-large-sized arteries, the nature of blood can be described as a macroscopic non-Newtonian fluid however, flow in smaller vessels requires treating human blood as a multiphase fluid containing 40-45% RBCs by volume. Simulations of blood at a cellular level provide a tool that allows exploration of the rheology, stress, and diffusion of individual suspended cells. In our approach, the blood plasma and hemoglobin are treated as Newtonian fluids with viscosities of 1.2 and 6 cP, respectively. The multiphase nature of the blood creates non-Newtonian flow effects, such as shear thinning, commonly observed in experiments. An image of multiple clusters forming in Hagen-Poiseuille flow is shown below. We have shown the ability to simulate thousands of deformable particles and capture non-Newtonian characteristics that agree well with experimental observations. Our approach has shown the ability to scale on as many as 65,536 computational cores of Argonne National Laboratory’s IBM Blue Gene/P. When computing the dynamics of 1,000’s to 100,000’s of deformable particles suspended in fluid, datasets can easily reach 1 terabyte or greater for a single simulation. As an example, one such simulation with 100,000 deformable spheres (each with 254 triangular surfaces) requires a LB fluid domain in excess of 2 billion lattice grid points. ![]() When fluid and solid data sets from CFD codes are O(100 GB), the use of distributed memory architectures is required for post-processing. |
